Or get the ARco book, ';The Homeschoolers Guide to Portfolio's and Transcripts';
Or get ';Homeschooling for Dummies';
Or any one of the many books about homeschooling High schoolers. Some should be available at your library.
There is even a software program called ';Transcript Pro.';
Also, talk to your parents about it, maybe they have something planned they haven't told you about.
You might want to look from some sample transcripts.
Usually it just has the school name and address. A contact name.
Then the time frame, courses taken, final grades
That's what they're really looking for
Grade 9, 2003
Biology 1 A
Alegbra 1 B
Spanish 1 A
Or the numeric score if it was used instead
They want to see what courses you took and how well you did in them
Technically aren't supposed to lie
If you took Geometry 1 and got a C or D the first time and took it again you are TECHICALLY supposed to document that.
In a real school transcipt it will say that
In your college transcripts if you get into trouble it will clearly state you were put on
ACADEMIC PROBATION FROM 2009-2010
Your employer or grad school, if they request these, will see that
Anyone can get your transcripts if you give permission in writing
Any school is allowed to ask for a copy if you apply for admission there
It is a permanent records of your academic history
When you make a job application you allow them to do a background check and they can ask for your transcipts to see if you do have a high school or college diploma as you so claim.
You need to talk to your parents about this. They are the only ones who can give you a transcript. They have to decide graduation requirements, even if it's after-the-fact, and put something together for you. Hopefully this will help you (your parents):

Like others have said, talk to your parents about it.
Definitely check in with mom and dad. It can be intimidating to jump into this but there is help! Texas Homeschool Coalition is a primary source to help with things like transcripts and portfolios. They have an online site and offer several resources in line with that. Also are you connected with a local support group? Usually there are knowledgeable people who can help you develop what you need. The Homeschool Tracker offers free downloadable software for you to use to keep records and transcripts. Check them out online.
First of all you are overstepping your bounds by asking this on here instead of speaking to your parents first. You need to go to them for this.
http://www.homeschooloasis.com (Barb Shelton's book High School Home School Form U La) has great info on high school. The book has awesome forms (some of which are online) for keeping track of what you've done, pacing yourself, making transcripts, graduation ideas and diplomas.
Talk to your parents.
